Hello ! I am back to my dream land! Had been away from my dream land for so many days! Now I am back! ^_^

Just finished a series title “The master of TAI CHI”. It is a kungfu tvb series, 25 episodes. I finished it in 3 day! Took so much of my times watching but personally, I like it very much. A lot of kungfu acting in it, very exciting and nice.. The kungfu moves are so graceful and ‘geng’!
Moreover, there are 2 handsome guys inside too! Which are Vincent Zhao and Raymond Lam.

Vincent Zhao acted as Mo Ma (巫馬) in the series

Raymond Lam acted as Tuen Hiu Sing (段曉星). This is him with the specs on. Looks very gentle man! He drools me off from the reality world.

Him without specs. Still handsome! ^_^ Well, in the series, he is the leader of the top-ranked Chong Lung Sect.

Him with another style of presentation. Looks very ….ermm…don’t know how to describe. Anyway, he looks good and handsome everytime. Hope I can have him as my boyfriend. I know it’s impossible, but, in my dream, is it possible?

Last but not least, a very beautiful snap shot of scenery in series. This is one of the scenery. There are so many more eye-catching scenery in the series~

Time to go to class now! Got to leave my dream land now. Sigh~

Ijyuuin, an intern, did a surgery on acute appendicitis.
Dr. Asada instructed him to do it by himself because he picked up the emergency.
It was his first surgery done all by himself. Of course, he was nervous. He never had an experience of doing the opearation. Everytime, he is only read from the book. Because this is the first time, so he did it according to what he read from the book. Unfortunately, the appendix wasnt at the place stated in the book. He couldnt find the appendix.

Dr. Asada came out with a name “Douglas pouch”.

Finally, he found it! So, removal of appendix was carried out.

Thus, REAL SURGERY is unlike TEXTBOOK EXAMPLES.
So, for surgeons, without experiencing actual operations, it is impossible to improve. –DR. ASADA -

Case TWO
Meishin University Hospital, ER department doesnt accept foreign patient. Further more the patient is in critical shock state. According the other dr. it was impossible to save the patient. The patient doesnt have the doesnt have health insurance, which menas hospital itself hav to pay for his treatment fees. BUt Dr. Asada still accept the case no matter what.

Patient’s blood pressure cannot be determined. THere may be excessive bleeding in the abdomen.
Patient was confirmed with excessive bleeding in his abdominal cavity.
Ultrasound examination showing the “ECHO FREE SPACE”

Abdominal region was cut opened. Blood filled the abdominal cavity, too much blood that they could see anything.
Blood was removed. Liver damage was diagnosed, with 3 injured regions, S4, S6, S8.
Treated with PRINGLE MANEUVER.

-Using vascular clamp to clip the portal triad and thus preventing blood flow into the liver.

-During the short time the blood flow is stopped.

-THe wounds on the liver was stitched up.

Dr. Asada stitched up the wounds very quickly, not even 5 minutes was spent for each wound.
WHen he wanted to stitched up the last wound, where liver needed to be lifted up, appear another wound which the intern didnt found out. This cause the abnormal bleeding to start again.
Dr. Asada searching for the wounds, but the time is running out. BP was dropping. to 40/22
Thus, Schrock shunt was used to buy more time to search for the wound.
By directly connecting the atrium sinistrum (left) to the liver vena cava, to create another alternative bloodflow pathway, thus providing extended time to locate the wound.
Bosmin 10cc was given. Bosmin is a trade name for Epinephrine.

ANother hidden wound was searched for. FOund it finally, repairing work was done. Operation was successful.

Case number 3

THe hospital was trying a new medicine, anti-carcinogen, TMT3 on a patient with end-stage of lung cancer.
It has very strong side effects, other hospitals have ceased using it. It destroy the cancer cells, but the patient’s life is endangered too.

The old patient, who was chosen for the trying out of the new medicine.

The doctor trying it out in the patient because she is untreatable with a life span 3 more months.
THe side effects are severe vomiting and difficulty in swallowing.
The degree of side effects are determined by the doctor himself, for example, the side effects maybe due to patient’s herself and not due to the medicine.
Furthermore, due to the complications that arises, cancer cells may slightly be reduced. But even if no change, that medicine will still be accepted.
For the pharmaceutical company, as long as there is a hospital that is willing to use the medicine. Whatever the outcome, the medicine will still be effective. And both of the hospital and pharmaceutical company will have all the benefits after approving the medicine for treatment.

Patient’s family members were not informed about the severe side effects, they were only told about the good effect of medicine. THat is why, they agreed to use the medicine.
But Dr. Asada told them the severe side effects, and her husband decided to stop the medicine, and transfer to another hospital, so that the patient could stop the suffering of the side effects and die peacefully, without any pain.

Hmmm… Hope I could find such a good marriage partners and live a happy life like them too….
Well.. Now it is just a DREAM…. When will this Dream became REALITY?

In the end, the patient died without suffering from any pain….
Even though, there is no treatment for Cancer yet, but at least, patient is not suffering from any pain before she leave the world….

Currently watching TMD (Team Medical Dragon). Nice Japanese series storying about a very outstanding surgeon named DR. ASADA (Asada Ryutaro) and also the team he had built, with the name TEAM MEDICAL DRAGON.

Story started with Dr. Kito bringing Dr. Asada to Meishin hospital to help her out for the Batista Thesis. If the thesis is successful, she can have the important position in Japan medical field.
Meishin University Hospital is a hospital which focus more on the benefit of themselves than the patient. Patient who has low survival chance, will be designate as ‘Entlassen’ means discharge from hospital and awaiting death. This is to avoid attempting low-success operations such as operations which requiring high skill

So, now, let’s watch and see how Dr. Asada is going to work in this kind of hospital, in where, he had done a lot of difficult operations and also succeed in all of them.

In every series, there are two or three medical cases which are very interesting to know about. I am sure there is a lot of things we can learn from the series.

The FIRST case.
Cardiogenic shock…. Cardiac tamponade. To not affect insurance ratings, doctors still have to treat hopeless patients. even if no treatment. to avoid the increase of hospital’s death toll, th patient is transferred to the Meishin University Hospital. To the patient’s family, dying in a well-equipped hospital, they will accept it more easily. This is a patient whose life does not matter. As long as you follow the procedure, the outcome of operation is not important.

Operation on cardiac tamponade.

Due to injuries, blood flows out of the myocardium causing compression in the heart. Operation requires opening up the myocardium and drain out the excess blood.

Patient’s heartbeat stopped. (ASYSTOLE)
The operation was successful but the patient died. Dr. Asada then took over the surgery and continue the surgery. According to him, it is not natural for the heartbeat to suddenly stop due to cardiac contusion. There must be other reason. Thus, we should investigate the real cause of the heart failure.
He grabbed the heart and looked for the cause, and found CARDIORrHExis, tear of the heart, because of that, the bleeding continues.

Using thumb to apply the slight pressure to remedy time needed to stitch the perforation. And at the same time, perform cardiac massaging. Gently and slowly like molding a ball, gradually increasing the pressure.
After a few minutes, the heart started to BEAT! Heart revived!

After that, treatment of cardiorrhexis was carried on..4-0 stitching tool.
Heart perforation is treated. Bleeding ceases. 3-0 stitching tool.

Do you think this kind of operation also happen in the real case? I am not sure.

SECOND case.
Miki, a first rate nurse.

She fainted suddenly. Breathing had stopped but the heart still beating. Based on these symptoms, her two lungs have simultaneously developed pneumothorax.

Due to formation of the holes on the lung lobes inside the lung thorax. Burst of air flows out and builds up pressure inside the lung thorax. No matter how hard you inhale, the lung is unable to berestored to its original shape. If both of her lung had reached this stage, her life will be endangered.
Dr. Kato Akira decided to call ambulance but according to him, if they were to wait for help, it would be too long, and even if help comes in time, there is risk of brain damage.

Thus, he borrowed a pen from dr. Kito remove the inner part of pen and break it into half. And stab it on both of her chest.

An emergency treatment by using a pen to pierce into her lung thorax in order to let the air inside escape and thus allowing the lungs to restore to its normal state. Such crude method, but if this action was not carried out, she may die.

Dr. Asada finally agree to work in Meishin hospital with the a condition, he can gather up his own MEDICAL TEAM. The first member in his team is Miki.

Batista Operation

Batista operation is named after a Brazilian surgeon Randas Batista, who invented this operation proceudure. The operation is used to treat enlarged heart muscle. Excising the enlarged heart muscle and stitching it back to make it smaller. A difficult operation.

Batista operation is one of surgery procedures used to treat over-enlarged heart muscles. As opposed to only having the option of heart transplant. It provides sufferers an improved alternative. It is becoming the preferred method and gaining popularity. Other than requiring an unparalled medical technology, there ar estill lies an assortment of difficulties. However recently after significant technology improvements, the surgery success rate is much higher

The bad guy character in the series… name will be revealed next time…
